[Troop351] Updated Med Forms Needed

All,

Scouts who are going to summer camp will need med form C filled out by a
doctor.  See the forms under "Are you going to camp?"

We have a mix of current forms and expired forms.  I hope to have a roster
soon so that you know what's needed.  These expire yearly, so if you have
not turned in a C form since last year before summer camp, then please work
on getting an updated form soon.

In addition to the regular medical forms, we will need to have a copy of an
insurance card and a new form for the middle tennessee council.  MTC
requires a Mental, Emotional, and Social Health (MESH) form.  I'll send out
a link to this once I have it.

The C form must be filled out by a doctor, but you should be able to do the
rest on your own.
