<div dir="ltr">All,<div><br></div><div>Scouts who are going to summer camp will need med form C filled out by a doctor.  See the forms under "Are you going to camp?"</div><div><a href="https://www.scouting.org/health-and-safety/ahmr/">https://www.scouting.org/health-and-safety/ahmr/</a></div><div><br></div><div>We have a mix of current forms and expired forms.  I hope to have a roster soon so that you know what's needed.  These expire yearly, so if you have not turned in a C form since last year before summer camp, then please work on getting an updated form soon.</div><div><br></div><div>In addition to the regular medical forms, we will need to have a copy of an insurance card and a new form for the middle tennessee council.  MTC requires a Mental, Emotional, and Social Health (MESH) form.  I'll send out a link to this once I have it.</div><div><br></div><div>The C form must be filled out by a doctor, but you should be able to do the rest on your own.</div><div><br></div><div>Josh</div></div>
