[Troop351] Med Forms C needed

The following campers need med forms C turned in.  Almost all of these need
updated A & B forms for the summer camp folder.  If a name is listed and
the forms were already turned in, you may need to turn in another copy.  We
have had several forms turned in that were put in camp specific bundles.
If this applies to you, please let me know.

We need A, B, and C.  C is to be filled out by a doctor.  Please bring
these forms and a copy of your insurance card if it has changed recently or
you have not previously submitted it.  You can turn them in when we pack
the trailer Saturday, or if you need to get those to me some other way, let
me know.

We will be packing the trailer Saturday morning 6:30-8:30.  Additional
emails to follow about packing the trailer and departure times.
